Book Synopsis Transition from Pediatric to Adult Healthcare Services for Adolescents and Young Adults with Long-term Conditions by : Cecily L. Betz

Download or read book Transition from Pediatric to Adult Healthcare Services for Adolescents and Young Adults with Long-term Conditions written by Cecily L. Betz. This book was released on 2019-11-30. Available in PDF, EPUB and Kindle. Book excerpt: This book offers essential information on interventions and actions that enable and promote transition experiences for adolescents and young adults. It provides guidance on appropriate strategies that bring together these groups and caregivers in the context of transition preparation, and those which optimize adolescents’ and young adults’ ability to self-manage their healthcare. The health care transition for adolescents and young adults has gained more attention given the improved survival rates, including for those with long-term conditions. It is now estimated that more than 90% of children diagnosed with a long-term condition will survive into adulthood. The significant change in survival requires concomitant services to facilitate adolescents’ and young adults’ successful transition to adult health care, therefore providers need education, knowledge and skills to support healthcare transition services. A recent US national survey demonstrated only 10% of parents/caregivers reported that their children received transition preparation services. This book is targeted at nurses and pediatric and adult health care providers of different disciplines seeking guidance on which interventions are available, how they can be used, advantages and challenges, and how best to promote a seamless transition for all adolescents and young adults with long-term conditions. It provides several examples of transition programmes and initiatives worldwide. There is an increasing focus on how nurses can support transition but little guidance on what is effective and what has been tried. This book will fill a gap by addressing all of these issues outlined above and by providing worked examples from leading nurse researchers and academics worldwide.

Book Synopsis Beyond Survival by : Elizabeth J. Wood

Download or read book Beyond Survival written by Elizabeth J. Wood. This book was released on 2006-12-30. Available in PDF, EPUB and Kindle. Book excerpt: One part theory (borrowed from business world), one part practice (including detailed case studies of the University of Pittsburgh and the University of Arizona), one part inspiration: Beyond Survival offers ideas about how academic libraries can not only survive in the short term, but take advantage of emergent opportunities by judiciously adopting the same organizational development tools and concepts espoused by the business world. While there is a great deal of anecdotal evidence that significant organizational changes are taking place in academic libraries, the literature suggests that most of these changes take the form of evolutionary, or incremental improvement. But what happens when libraries find themselves in a society characterized by increased information availability compression of time and space, and growing turbulence and unpredictability? These are conditions with which the business world has been grappling for years, conditions that require not an evolutionary approach, but nimbleness and rapid response. One part theory (borrowed from business world), one part practice (including detailed case studies of the University of Pittsburgh and the University of Arizona), one part inspiration, Beyond Survival shows you how the transition tactics and strategies developed by businesses can be adapted to academic libraries. By judiciously adopting the same organizational development tools and concepts espoused by the business world, academic libraries can not only survive in the short term, but can take advantage of emergent opportunities to ensure long-term excellence.

Book Synopsis Researching Quality in Care Transitions by : Karina Aase

Download or read book Researching Quality in Care Transitions written by Karina Aase. This book was released on 2017-09-15. Available in PDF, EPUB and Kindle. Book excerpt: This book is concerned with the complexities of achieving quality in care transitions. The organization and accomplishment of high quality care transitions relies upon the coordination of multiple professionals, working within and across multiple care processes, settings and organizations, each with their own distinct ways of working, profile of resources, and modes of organizing. In short, care transitions might easily be regarded as complex activities that take place within complex systems, which can make accomplishing high quality care challenging. As a subject of enquiry, care transitions are approached from many research, improvement and policy perspectives: from group psychology and human factors to social and political theory; from applied process re-engineering projects to exploratory ethnographic studies; from large-scale policy innovations to local improvements initiatives. This collection will provide a unique cross-disciplinary and multi-level analysis, where each chapter presents a particular depth of insight and analysis, and together offer a holistic and detail understand of care transitions.
