Book excerpt: Excerpt from The Medical Press of Western New York, Vol. 4: January-June, 1889 If we have exhausted all our resources and no clue is discovered as to the locality of the urethra, nothing remains but a hunt for it in the mass of cicatricial tissue in the bottom of the wound, and if perineal section, with a guide is one of the simplest operations in surgery, far, very far different is it when you have no guide. You have only the apex of the prostate to aim for and to try and keep in the middle line, which is just about as easy as walking a slack rope. With your finger in the rectum its tip against the prostate, repeated short incisions through the cicatrix may reach the urethra, or it may be that the cicatrix must be dissected out en masse. Some surgeons advise that this be done anyhow, after you have divided= the stricture. The wound being a large one may well be closed, except the lower part, and the after treatment managed as before, and it goes without saying, that through out the whole time scrupulous attention to cleanliness must be shown.
